Important Information for Patients
ReVITALized Wellness, PLLC is committed to providing professional, ethical, and accessible mental health care. Please review the following policies and frequently asked questions before requesting an appointment.
Q: What is mental health?
A: Mental health relates to how we think, feel, cope, and connect with others. Just like physical health, mental health can change over time and requires care, attention, and support.
Q: Is it normal to struggle with mental health?
A: Yes. It is very normal. Everyone experiences stress, anxiety, sadness, or feeling overwhelmed at different points in life. Struggling does not mean you are weak. It means you are human.
Q: How do I know if what I’m feeling is serious enough to get help?
A: If what you are experiencing is affecting your daily life, relationships, work, sleep, or sense of self, that is reason enough to seek support. You do not need to be in crisis to deserve care.
Q: Are all appointments virtual?
A: Yes. All services at ReVITALized Wellness are provided via secure telehealth.
Q: Can documentation be requested later?
A: Completion of documentation requires an established treatment relationship and sufficient clinical history. Requests are considered on a case-by-case basis and are not guaranteed.
Q: When should I seek immediate help?
A: If you are thinking about harming yourself, feel unsafe, or believe you may be in immediate danger, seek emergency support right away. You deserve help in that moment.
Q: Can I attend my telehealth appointment while driving?
A: No. Telehealth appointments require your full attention and must take place while you are in a stationary, safe location. If you are driving or unable to remain parked for the duration of the visit, the appointment will need to be rescheduled. Video must remain on throughout the session.
Q: Who is eligible for services?
A: ReVITALized Wellness serves adults 18 years and older who are physically located in Texas at the time of their appointment.
Q: Do you see children or adolescents?
A: No. At this time, ReVITALized Wellness provides services to adults only.
Q: How do I request an appointment?
A: All new patients must complete a prospective patient request form. Appointment availability is shared after your information is reviewed.
Q: Will the provider complete FMLA, disability, or work accommodation paperwork during my evaluation?
A: No. The provider does not complete FMLA, disability paperwork, work accommodations, legal documentation, or emotional support letters during the initial evaluation. Initial visits are for assessment purposes only.
Q: What if I’m worried about someone else’s mental health?
A: Check in with them, listen without judgment, and encourage them to seek professional support. You do not need to have all the answers to be helpful.
