ReVITALized Wellness, PLLC provides tele-health mental health services for adults (18+) located in Texas. All new patients must complete a prospective patient request before an appointment can be scheduled.
This process helps ensure we are the right fit for your care needs and allows us to review availability.
Care is provided by Dr. Charmeka Lipscomb, DNP, MBA, MHA, FNP-C, PMHNP-BC, a board-certified nurse practitioner with advanced training in both family and psychiatric mental health care.

Dr. Charmeka Lipscomb looks forward to expanding availability and serving patients on a full-time basis in the future.

Fees & Policies (Summary)

Same-day cancellations or missed appointments will result in a $75.00 fail-to-cancel / no-show fee

FMLA, disability paperwork, work accommodations, and emotional support letters are not completed during initial visits

We do not provide ESA letters but here are some options.
