ReVITALized Wellness, PLLC provides Telehealth mental health services for adults (18+) located in Texas. We accept select insurance plans as well as self-pay options.

Insurance coverage and benefits vary by plan. Patients are responsible for understanding their individual coverage, including copays, deductibles, and any balances not covered by insurance.

Comprehensive mental health assessment.
Medication Management Follow-Up:
$150.00
Ongoing visits focused on monitoring symptoms and adjusting treatment when appropriate.

Payment is due at the time of service.
Cancellation & No-Show Policy
Fail-to-Cancel / No-Show Fee:
$100.00

Appointments that are missed or canceled the same day without adequate notice will result in a $75.00 fee, regardless of visit type. This fee applies to all scheduled appointments.
